Taking Time for Every Patient
If you are new to Mountainside Holistic Clinic, please follow the following steps to better assist you with making your initial appointment with us.
Please call our office at (540) 745-4654, during our open, scheduled hours, to see if this service is needed, or to schedule your initial visit & evaluation.
When contacting our office please have the following information available:
- Name
- DOB
- Phone Number
- Email Address
- Chief health concern
Once you have scheduled your appointment, we will email the appropriate intake forms to complete online!
Please complete all your paperwork prior to your first appointment.
We recommend arriving 10 minutes prior to your appointment time.
Additionally, bring to your appointment or email to our office all supporting medical records including any lab work that relates to your health concern(s). Dr. Jess would like you to bring any/all supplements and prescription meds (if any) to your first appointment as well.
Cancellation Policy: Please understand your appointment time is reserved for you. We recognize there may be occasions when you need to cancel or reschedule an appointment. If you need to make any changes to your appointment for any reason, we require a minimum of 24-hour advanced notice to avoid a missed appointment fee of $50 for all followups, and $100 for all new patient appointments. We appreciate your understanding regarding this policy.
